If the concentration of a drug in the blood is too low, the drugs effectiveness will be low; if the concentration is too high, the drug may cause toxic-effects. The "therapeutic index" is the optimal blood concentration of the drug, where toxic-effects are low while effectiveness is high.
In order to achieve this "therapeutic index" consistently, it is important to space your doses correctly throughout the day. This is very important for drugs which have narrow therapeutic indexes, i.e. time-sensitive drugs.
